Registration Requirements

Immunization/Physical Exam:

The Texas Department of Protective and Regulatory Services requires that each child have a physical examination before being accepted in care. A form must be completed and signed by the person doing the exam (doctor, Health Department).

Health statements are required within the first seven days of attendance.

We are also required by the State to keep on file a current immunization record for every child not enrolled in public school. Immunizations records are required on or before the first day of care.

Tuberculosis is not required at this time by Amarillo Health Officials.

First-time enrollees who are four years of age or older and all children enrolled in programs who are four years of age by September 1st of each year will be screened for possible vision and hearing problems prior to completion of the first semester of enrollment or within 120 calendar days of enrollment, whichever is longer, or present evidence of screening conducted one year prior to enrollment.

We have certified a screener that comes to the center and conducts an individual screening on all children four years or older who are not enrolled in a public school.
